Salary Range: $100,000 – $115,000 annually, commensurate with experience, qualifications, and demonstrated leadership performance.

What You'll Do
- Demonstrate commitment to NLCS’ core values in all aspects of your work
- Support NLCS policies, procedures, guidelines, and code of conduct standards

- Foster a positive work environment that promotes team member engagement and satisfaction
- Coordinate individualized plans of care using an interdisciplinary team approach
- Review and evaluate resident MDS assessments and care plans to ensure proper implementation
- Participate in resident care plan conferences
- Maintain the MDS schedule within required time frames and communicate pending assessments to relevant directors
- Generate and distribute the monthly MDS calendar
- Complete and audit MDS assessments, ensuring all required signatures are obtained
- Transmit electronic copies of assessments to the Centers for Medicare and Medicaid Services (CMS) and reconcile all transmissions
What We're Looking For
- Current RN license in the State of Maryland
- MDS certification or proven experience completing MDS assessments
- Minimum 2 years of clinical experience. Minimum of 1 year of experience in a long-term care setting required
- Prior experience as an MDS coordinator is strongly preferred
- Must possess strong written and oral communication skills; able to maintain confidential information
- Must be proficient in Microsoft Office
- Strong organizational and analytical skills with excellent attention to detail.
