About the role
Liquid Personnel is recruiting an Adult Social Worker to join its client’s Preparing for Adulthood - Learning Disabilities & Autism Team, based in Rochdale.
What will your responsibilities be?
- Conducting Care Act assessments: Evaluate young people (usually 17+) to determine eligibility for adult social care services.
- Coordinating transition planning: Work with schools, families, and professionals to ensure smooth movement from children’s to adult services.
- Supporting independence and community inclusion: Help individuals develop daily living skills, access housing options, and engage in employment or training pathways.

Qualifications and Experience
To be successful in this role you must have.
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
