About the role
Liquid Personnel is recruiting an Adult Social Worker to join its client’s Hospital team, based in Manchester.
What will your responsibilities be?
- Managing hospital discharges - completing timely Care Act assessments, coordinating safe discharge plans and ensuring appropriate community support is in place.
- Liaising with reablement and MDTs - working closely with therapy teams, ward staff and external providers to progress discharge pathways and reduce delays.
- Undertaking hospital‑based social work tasks - addressing safeguarding concerns, assessing risk, updating support plans and supporting complex cases within a fast‑paced acute setting.

Qualifications and Experience
To be successful in this role you must have;
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
