Interventions can be delivered through a mix of home visits, telehealth, telephone, clinic visits or even meeting with patients and carers in another community location, depending on clinical need and patient/carer preferences. The focus of treatment is to reduce the risk of representation, with a focus on symptom management, falls prevention, compliance with discharge recommendations
