Your new role

As the Nurse Navigator within Chronic Disease, Community and Oral Health Services, you will provide advanced clinical leadership and coordinate person-centred care for adults with complex chronic conditions, supporting improved health outcomes and reducing avoidable hospital presentations and readmissions. Key responsibilities include:

  • Coordinate complex patient care across primary, secondary and tertiary healthcare settings, building effective partnerships with patients, families, carers and multidisciplinary teams to ensure seamless care transitions.

About you

You are an experienced and motivated nursing professional with advanced clinical expertise, a commitment to person-centred care and the ability to navigate complex healthcare systems to achieve positive outcomes for patients living with chronic disease. You will bring:

  • Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia and a tertiary qualification in nursing, with relevant postgraduate qualifications or evidence of enrolment highly desirable.
  • Advanced knowledge and clinical experience in complex chronic disease management, contemporary nursing practice and the application of primary healthcare principles to improve patient outcomes.
