About the role

Join the Patient Access and Clinical Effectiveness (PACE) team at Caboolture Hospital as a Clinical Nurse Discharge Planner, where you will play a pivotal role in coordinating complex patient discharges and ensuring seamless transitions across healthcare services. Using advanced clinical knowledge and assessment skills, you will work closely with patients, families, multidisciplinary teams and external service providers to facilitate safe, timely and patient-centred discharge outcomes.

In this role, you will provide expert guidance regarding community care services, manage referrals for ongoing care, and support patients with complex healthcare needs. You will contribute to reducing avoidable hospital readmissions through comprehensive discharge planning, patient education, care coordination and leadership within a collaborative healthcare environment.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Coordinate complex discharge planning activities to support safe and effective transitions of care across acute, subacute, community and primary healthcare services.
  • Apply advanced clinical assessment, planning, implementation and evaluation skills to deliver high-quality patient-centred care.
  • Provide expert advice on community services, ongoing care pathways and referral processes to support optimal patient outcomes.
  • Collaborate with multidisciplinary teams, patients, families and external providers to facilitate integrated and coordinated care.
  • Lead quality improvement initiatives, provide clinical support and education, and contribute to evidence-based practice within the PACE service.

Benefits when working for us

  • Rewarding career and development opportunities across a wide range of clinical and non-clinical areas
  • Value driven organisation which provides a work environment that is safe, satisfying, flexible, and promotes a healthy work-life balance
  • Flexible working arrangements and competitive salary rates with annual incremental increases
  • Benefit from a higher than standard employer contribution to Superannuation of up to 12.75% and access to salary packaging
