About the role

What your day could look like

As a Clinical Nurse – Community Health, you'll play an important role in improving health and wellbeing across the community. From providing advanced clinical assessment and care planning to supporting people to better manage their own health, you'll work alongside multidisciplinary colleagues to deliver care that makes a meaningful difference close to home.

Every day, you'll contribute by:

  • Delivering comprehensive assessments, care coordination and evidence-based interventions that support positive health outcomes for individuals and families.
  • Building trusted partnerships with patients, carers, community services and healthcare professionals to provide connected, responsive care.
  • Leading and influencing safe, high-quality practice through clinical expertise, mentoring and continuous improvement activities.
  • Promoting health, wellbeing and self-management through education, early intervention and preventative care initiatives.

Let's Talk About You

You're a confident and compassionate nurse who combines clinical expertise with genuine care for people and communities. You bring sound judgement, strong communication skills and a collaborative approach, helping others feel supported while navigating complex health needs. Whether you're coordinating care, mentoring colleagues or advocating for better outcomes, you'll take pride in making a lasting impact. Your approach reflects the values of Compassion, Integrity, Dignity, Innovation and Courage, and your commitment helps create healthier communities now and into the future.

Here's what will help you thrive in this role:

  • Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
  • Advanced assessment, care planning, case management and clinical decision-making skills.
  • Experience working collaboratively across multidisciplinary teams and community-based healthcare settings.
  • A commitment to quality, safety, continuous improvement and the professional development of yourself and others.

You'll stand out from the crowd if you have a qualification in rural and isolated practice, immunisation, and/or postgraduate study relevant to community health.

Together, you'll help deliver safe, connected and compassionate care that strengthens the health of the community.
