About the role

Come Join Us!

Our Mobile Primary Care Team is seeking a dedicated and compassionate Physician Assistant or Nurse Practitioner who is energized by the opportunity to make a meaningful impact in the lives of seniors. We are looking for a clinician who thrives in an autonomous environment, takes ownership of their practice, builds trusted relationships, and approaches patient care with both clinical excellence and an entrepreneurial mindset. Whether you are seeking a part-time opportunity or are ready to commit full-time, we offer the flexibility to grow your practice and career in a way that aligns with your professional goals.

This role is ideal for a self-starter who is professional, accountable, adaptable, and motivated to help grow and strengthen the communities they serve. While you will enjoy the independence of managing your own patient panel, you will also be supported by a collaborative team committed to your success.

We look forward to meeting you and putting a heart of GOLD back in the Golden Years!

At Lykos Medical, part of Brigade Health and its Affiliates, we're redefining traditional healthcare by bringing medical expertise directly to where seniors live: their homes and communities. Brigade has grown to serve hundreds of senior living communities across multiple states, earning recognition for delivering personalized, relationship-centered care.

As a Physician Assistant or Nurse Practitioner, you'll join a purpose-driven organization that:

  • Empowers independent clinicians who take initiative, solve problems, and drive positive outcomes for their patients and communities.
  • Provides continuum-based care by integrating primary care, psychiatry, palliative care, neurology, and home-based services.
  • Fosters a culture of professionalism and accountability, where trust, integrity, and follow-through are highly valued.
  • Values partnership and collaboration, creating opportunities for mentorship, leadership development, and long-term career growth.
  • Supports entrepreneurial thinking, encouraging providers to build relationships, expand community engagement, and contribute to the growth of our practice.
  • Combines clinical excellence with compassion, ensuring every patient receives personalized, high-quality care.

What Makes This Role Unique

  • Autonomy with Purpose: Enjoy the flexibility and independence of managing your practice while being supported by experienced clinical and operational leaders.
  • Entrepreneurial Opportunity: Build strong relationships with patients, caregivers, and community partners while helping expand access to quality healthcare services.
  • Dementia & Senior Care Expertise: Develop advanced knowledge in dementia care, chronic disease management, and whole-person senior health.
  • Growth-Oriented Environment: We are seeking providers who view challenges as opportunities, embrace continuous learning, and want to grow alongside an expanding organization.
  • Collaborative Support Network: Work with dedicated specialists and interdisciplinary partners to provide exceptional patient outcomes.

What You'll Bring

  • Active, unrestricted NP or PA license in the state of practice.
  • Minimum 1-2 years of clinical experience (geriatrics, primary care, home-based care, or value-based care preferred).
  • A highly professional, dependable, and trustworthy approach to patient care.
  • Strong clinical judgment and the ability to work independently with confidence.
  • Excellent relationship-building and communication skills.
  • An entrepreneurial mindset with a passion for growing provider-community relationships.
  • Adaptability, accountability, and a commitment to delivering exceptional patient experiences.
  • Valid driver's license and reliable transportation.
